Terms Beginning with the Letter "F"


Family Unit is the covered Employee and the family members who are covered as Dependents under the Plan.  

Family Unit Limit
is the dollar amount shown in the Schedule of Benefits. When this limit has been incurred by the Family Unit toward their Calendar Year deductibles, the deductibles of all members of that Family Unit will be considered satisfied for that year.  

Forfeiture
is the amount of money in the Health Care Reimbursement Fund at the end of the Plan Year after processing all the claims subsequent to the Plan Year. This money will be forfeited and credited to the benefit plan surplus.  

Formulary
means a list of prescription medications compiled by a third party payer of safe, effective therapeutic drugs specifically covered by this Plan.

Foster Child
means an unmarried child under the limiting age shown in the Dependent Eligibility Section of this Plan for whom a covered Employee has assumed a legal obligation. All of the following conditions must be met: the child is being raised as the covered Employee’s; the child must primarily depend on the covered Employee for support; the child lives in the home of the covered Employee; and the covered Employee may legally claim the child as a federal income tax deduction.

A covered Foster Child is not a child temporarily living in the covered Employee’s home; one placed in the covered Employee’s home by a social service agency which retains control of the child; or whose natural parent(s) may exercise or share parental responsibility and control.
